HOME CARE SERVICES

The Visiting Nurse Association of Wallingford, Inc., is a non-profit, public health and home care Agency. It is Medicare and Medicaid certified and licensed by the State of Connecticut. Home care services are available 24 hours a day, seven (7) days a week.


Source of Payment:

The Agency contracts with Medicare, Medicaid, numerous private insurance companies including Aetna, Ancillary Care Management (ACM), Anthem Blue Cross/Blue Shield, Community Health Network of CT., Connecticare, First Choice, Great Western Healthcare, Healthnet, Oxford Health Plans, Senior Whole Health, United Health Care, numerous HMO's, Area on Aging of South Central Connecticut, Area on Aging of Western Connecticut, and Connecticut Community Care.


Service Descriptions:

  • Curls on Wheels provides homebound clients with hair care services including cuts, washes, sets, and permanents.

  • Emergency Alert Systems offer a 24 hour safety net that provides security and increases independence. By touching a call-button, the client is in voice contact with a trained professional who will arrange emergency assistance and will remain on the line until the problem has been resolved.

  • Home Health Aides provide personal care to homebound clients. By administering baths, assisting with therapy, or providing personal care, home health aides enable clients to return to and remain at home more comfortably and safely after a hospitalization.

  • Homemakers assist with light home management activities when the primary household manager is unable to provide these services. Through such activities as caring for family members, preparing meals, shopping, assisting with laundry, and light housekeeping, homemakers help families remain together at home during periods of crisis or sickness.

  • Medical Social Workers assist clients and families to identify and address social, emotional, and financial difficulties which have resulted from health problems and/or handicaps. Social workers may arrange for admission and/or discharge from hospitals or convalescent homes, provide information regarding Medicare, Medicaid, fuel assistance and other community resources, and prepare families and individuals for changes in their daily lives.

  • Occupational Therapists evaluate and monitor clients in order to modify their environment and/or re-teach them to perform activities of daily living.

  • Palliative Care is available to clients who have a terminal diagnosis. The program focuses on control of pain and other symptoms while providing emotional and spiritual support, thereby providing the client and family with maximum comfort and control of their situation.

  • Physical Therapists evaluate and monitor a client's ability to use muscles and other body parts following a disease or injury in order to determine exercises which will restore function and minimize disability.

  • Registered Dieticians are available on a consultation or visit basis to assist with diabetic and other special diet planning and instruction. Nutrition assessments including medication and food interactions, caloric monitoring, and supplemental requirements are provided.

  • Registered Nurses serve as liaisons between clients and their physicians, assess and monitor clients' conditions, determine plans of care, perform skilled procedures, and teach clients and/or their caregivers to perform simple medical procedures so they can remain at home and attain maximum recovery.

  • Speech Therapists assist clients to restore or improve their ability to speak and/or communicate.
